1. Acceptance of Terms
By accessing or using FormFrog ("Service", "we", "our") at formfrog.app, you agree to be bound by these Terms of Service. If you do not agree, do not use FormFrog. These terms apply to all users, including free and paying subscribers.
2. Description of Service
FormFrog is a web-based form builder that lets you create, publish, and collect responses to forms. Features include conditional logic, AI-assisted form generation, drop-off analytics, team collaboration, and custom domains. We offer a Free plan and a Pro plan with additional features.
3. Account Registration
You must create an account to use FormFrog. You agree to provide accurate information and keep your credentials secure. You are responsible for all activity that occurs under your account. Notify us immediately at support@formfrog.app if you suspect unauthorized access.
You must be at least 13 years old to use FormFrog. By creating an account you represent that you meet this requirement.
4. Acceptable Use
You agree not to use FormFrog to:
- Collect sensitive personal data (government IDs, financial information, health data) without appropriate legal basis
- Send spam, phishing, or deceptive forms
- Collect data from minors without verifiable parental consent
- Violate any applicable law or regulation
- Infringe intellectual property rights of others
- Attempt to reverse-engineer, overload, or disrupt the service
- Resell or sublicense access to FormFrog without written permission
We reserve the right to suspend or terminate accounts that violate these rules without notice.
5. Your Content
You retain ownership of the forms you create and the responses you collect. By using FormFrog you grant us a limited license to store and process your content solely to operate the service. We will not access, sell, or share your form content except as required to provide the service or as required by law.
You are responsible for ensuring you have the legal right to collect data from your respondents and that you comply with applicable privacy laws (e.g. GDPR, CCPA).
6. Intellectual Property
The FormFrog name, logo, UI, and underlying code are owned by us and protected by applicable intellectual property laws. You may not copy, reproduce, or create derivative works from our platform without written permission.
7. Subscription & Billing
FormFrog offers a Free plan with core features and a Pro plan billed monthly or annually. Prices are shown on our Pricing page.
Subscriptions renew automatically at the end of each billing period. You can cancel at any time from your Account page. On cancellation, you retain Pro access until the end of your current billing period, after which your account reverts to the Free plan.
We reserve the right to change pricing with at least 30 days' notice to existing subscribers.
8. Refund Policy
Annual plan
If you are on an annual plan and are unsatisfied within the first 7 days of your initial subscription, contact us at support@formfrog.app for a full refund. After 7 days, annual subscriptions are non-refundable.
Monthly plan
Monthly subscriptions are non-refundable. You may cancel at any time to prevent future charges; you keep Pro access through the end of the billing period you have already paid for.
Refunds are issued to the original payment method and typically appear within 5–10 business days depending on your bank.
9. Chargebacks
If you initiate a chargeback or payment dispute for a charge that we believe is valid, we reserve the right to suspend or permanently terminate your account immediately. We encourage you to contact us at support@formfrog.app to resolve billing issues before filing a dispute — we are happy to help.
10. Disclaimer of Warranties
FormFrog is provided "as is" and "as available" without warranties of any kind, either express or implied, including but not limited to implied warranties of merchantability, fitness for a particular purpose, or non-infringement. We do not warrant that the service will be uninterrupted, error-free, or free of harmful components.
11. Limitation of Liability
To the maximum extent permitted by law, FormFrog and its operators shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the service, including loss of data, revenue, or profits. Our total liability for any claim related to the service is limited to the amount you paid us in the 12 months preceding the claim, or $50 USD, whichever is greater.
12. Termination
You may delete your account at any time. We may suspend or terminate your account at our discretion if you violate these Terms, with or without notice. Upon termination, your right to use the service ceases immediately. Sections 5, 6, 9, 10, 11, and 13 survive termination.
13. Governing Law
These Terms are governed by and construed in accordance with applicable law. Any disputes shall be resolved through good-faith negotiation before pursuing other remedies. You agree to resolve disputes individually and not as part of a class action.
14. Changes to Terms
We may update these Terms from time to time. We will notify you of material changes by email or in-app notice at least 14 days before they take effect. Your continued use of FormFrog after changes take effect constitutes acceptance of the updated Terms.
